The current development version 0.7.0.3 already has user defined formats 
(and templates) which can be applied to the current selection, so if you 
need custom html tags or the like this should already be possible.

But as I don't know your extensions I can only speculate if that is what 
you need or if you need more complex things. If it doesn't fit your 
needs please describe in more details, what goal you want to achieve 
with your customization (or what MediaWiki extensions you use).

If you test the 0.7.0.3 please be aware that some users had trouble 
(firefox hanging), so a new profile for testing may be a good idea (and 
knowing the "safe mode" to be able to uninstall the extension).
