I use something like this:

var menuitem = document.createElement("menuitem");
menuitem.setAttribute("class", "menuitem-iconic");
menuitem.setAttribute("label", label);
menuitem.setAttribute("src", url);

#YourMenulistID .menu-iconic-icon {
   display: block
}
