Possible reasons:

Some instances of firefox are still running. Make sure all
firefox windows are shut down. Use the task manager to make
sure that there are no processes called firefox.exe running.

Some of the subdirectories in your profile directory are set
to readonly. Does this problem happen with other extensions
or only with Flashblock?
