Congratulations on generating a Revocation Certificate _prior_ to losing
the passphrase.  :-D

Now, simply Import the Revocation Certificate into Your Keyring and then
Upload the revoked Key to the same Keyserver as before.

Then You are done; except for generation and Upload of a New Key.  ;)
