The extension manager API changed.  (I really don't understand these 
people; if you need a new interface, create a new interface, don't break 
the existing interfaces.)

Try this version, which should work for either version of the API. 

    * I also added a refresh when changing toolbars
    * I suppressed the 'Highlight' menu item
    * On Suite installations, it provides the choice of Profile or
      Application installation.

Still broken:
Uninstall doesn't work on Seamonkey, because of a change to the overlay 
database.
