[Clippings] Shortcut keys don't work?

First off, which application did you install Clippings in? Is this
happening in Firefox or Thunderbird?
It doesn't matter whether a clipping is in a folder or not; as long as
a shortcut key was assigned to a clipping, it *should* work as you
described.
Invoke Clippings Manager, look for the clipping with the assigned
shortcut key and confirm that the Shortcut Key drop-down field shows
the key that you assigned to the clipping.
When pressing CTRL+ALT+V, do you see a dialog box with this message:
"Press the shortcut key of the clipping you want to paste. To Cancel,
press Esc"? CTRL+ALT+V is enabled when a web page text box is focused
(Firefox) or you are composing an email or newsgroup message
(Thunderbird). In any other situation, CTRL+ALT+V will be ignored.
I would suggest disabling all your Firefox/Thunderbird extensions
except Clippings, and try again. Perhaps another extension is
interfering with CTRL+ALT+V.
Since Clippings is a Firefox and Thunderbird extension, it will only
work within either of those two apps. It will not work in other
desktop applications like Safari, Pages, MS Word, etc.
