I've used this feature my own for a long time, but I never encounter
such problems. You settings are OK. I don't know if it's a conflict
with other extension. Can you give me more informations?

If "Force middle clicked item opened in current tab" is disabled, a
bookmark item will be opened in new tab when middle clicked, no matter
the current tab is blank or not. It's the default behavior of Firefox.
If you want to open it in current blank tab, just left click the item.
Click2Tab will open bookmark item in current tab if it's blank.

Yes...I prefer external links to be opened in current tab if it's
blank.But currently this feature is not closely related to the current
features of Click2Tab. The main idea of Click2Tab is to open
bookmark/history item and cached closed tabs into new tabs. Reusing of
current blank tab is a necessary feature, but currently I can't make
all the tab opening behavior work this way. I just hope you can
understand it.

Maybe I will make a new extension that can do this later...:)

I've contacted with the author of TCO before. He has promised me that
he will add support to Click2Tab in the next release of TCO. So please
be a little patient. :)
