I used Add Bookmark Here Under Firefox 2, but since it's not been updated for FF3 (and I had issues bumping it), I went to Add Bookmark Here 2 -- which doesn't seem to allow the behavior Marco is talking about. Therefore, I think the add duplicate solution needs to be integral to BookmarkDD.
